MDADVICE.COM SWEEPSTAKES OFFICIAL RULES

The MDAdvice.com Sweepstakes ("Sweepstakes") is produced by The Online Medical Network Inc. and MDAdvice.com, 151 West Passaic Street, Rochelle Park, NJ 07662, ("MDAdvice.com").

ELIGIBILITY. VOID WHERE PROHIBITED. NO PURCHASE NECESSARY.

All federal, state and local laws apply. This Sweepstakes is open to all residents of the U.S. (excluding Florida), District of Columbia, and Canada (excluding Quebec) who are 18 years of age or older. Void where prohibited by law. Employees of MDAdvice.com, its associates, subsidiaries, dealers or retailers, advertising and promotion agencies and their families are not eligible.

The sweepstakes is limited to people who have email addresses since winners are notified by email.

HOW TO ENTER

You can enter by answering our online survey.

All of your Sweepstakes entries will be disqualified, at the sole discretion of MDAdvice.com, if you attempt to disrupt the Sweepstakes or circumvent the terms and conditions of these Official Rules. And, if either of the above occurs, MDAdvice.com reserves the right to remedy any such action, disruption, or circumvention in a manner, determined solely by MDAdvice.com, that is fair and equitable to Sweepstakes participants.

Only one entry is permitted per household.

PRIZES

The Grand Prize is $300 US Dollars. The Second Place Prize is $200 US Dollars. The Third Place Prize is $100 US Dollars.

Prizes are not transferable or assignable. Taxes on prizes are solely the responsibility of the winner.

Odds of winning depend on the number of eligible entries received.

Winners will be selected by random drawing from among all eligible entries received. Random drawings will be held under the supervision of MDAdvice.com, whose decisions are final and binding. Odds of winning depend on the total number of eligible entries received for the drawing. The winner will be notified by email. Winners will have up to 14 days from the date of attempted notification to respond to the notification. MDAdvice.com is not responsible for and shall not be liable for late, lost, misdirected, or unsuccessful efforts to notify winner.

Each winner will be required to provide MDAdvice.com with proof that the winner is the authorized account holder of the email address associated with the winning entry. If a selected winner cannot be contacted, is ineligible, or fails to claim a prize, an alternate winner will be selected. Winner consents to the use of his/her name and likeness for publicity and commercial purposes without further compensation unless prohibited by law.

CONDITIONS

By entering, participants agree to be bound by these complete Official Rules and the decisions of MDAdvice.com, which shall be final. All federal, state, and local laws apply. MDAdvice.com, Sweepstakes sponsors and the employees, officers, directors, shareholders, agents, and representatives of MDAdvice.com and its parent companies, affiliates, subsidiaries, and advertising, promotion and legal advisors are not responsible for and shall not be liable for: (i) late, lost, delayed, damaged, misdirected, incomplete, illegible, or unintelligible entries; (ii) telephone, electronic, hardware, or software program, network, Internet, or computer malfunctions, failures, or difficulties; (iii) errors in transmission; (iv) any condition caused by events beyond the control of MDAdvice.com that may cause the Sweepstakes to be disrupted or corrupted; (v) any injuries, losses, or damages of any kind caused by a prize or resulting from acceptance, possession, or use of a prize, or from participation in the Sweepstakes; or (vi) any printing or typographical errors in any materials associated with the Sweepstakes.

MDAdvice.com may prohibit you from participating in the Sweepstakes, if at the sole discretion of MDAdvice.com, you repeatedly show a disregard for these Sweepstakes Official Rules, or act (1) in an unsportsmanlike manner; (2) with an intent to annoy, abuse, threaten, or harass any other sweepstakes particpant or MDAdvice.com representative; or (3) in any other disruptive manner.

MDAdvice.com may terminate the Sweepstakes early at its own discretion; provided that all prizes will still be awarded. Winners will be drawn from entries received prior to the termination of the Sweepstakes.

Computer entries will be deemed to have been submitted by the authorized account holder of the email address submitted at the time of entry. "Authorized account holder" is defined as the natural person who is assigned to an e-mail address by an Internet access provider, online service provider, or other organization (e.g., business, educational institution, etc.) that is responsible for assigning e-mail addresses for the domain associated with the submitted e-mail address.
